the errors are caused

When someone visits your site/forum via http://www.phatcode.net/~subxero/forum/ instead of http://subxero.phatcode.net/forum/

You can either change your code that determines SITE_ROOT (looks like it generates an extra ../ as it is now) or put a redirect in your htaccess to force visitors to use the subdomain.
